Work-up includes MRI within 24 hours to rule out infarct,

carotid ultrasound, and endarterectomy for blockage greater than 70%.

Treatment includes antihypertensives, statins, and

81-mg aspirin.

There are no benefits with Aggrenox, clopidogrel, or ticagrelor over aspirin. The USPSTF now recommends 81-mg aspirin for primary prevention in 50- to 59-year-olds with a 10% 10-year CVD risk and without bleeding risk.
