Non-palpable

Non-palpable purpura indicates non-inflammatory bleeding.

Flat purpura suggests that blood has leaked from vessels without significant inflammation. The causes fall into three main categories: problems with platelet number or function, problems with clotting factors, or problems with vessel wall integrity itself.

Common causes of non-palpable purpura:

  • Thrombocytopenia (ITP, TTP, bone marrow failure, medications)
  • Platelet dysfunction (uremia, aspirin, inherited disorders)
  • Clotting factor deficiencies (hemophilia, von Willebrand disease, vitamin K deficiency, liver disease)
  • Vessel fragility (aging/senile purpura, steroid use, scurvy, Ehlers-Danlos syndrome, amyloidosis)
  • Trauma

Clinical implications:

Focus on CBC, coagulation studies, and platelet function testing.
